This webinar will introduce long-term care professionals to the concept of the age-friendly nursing home intended to simplify care, reduce burden, and improve outcomes for older adults. Attendees will learn the 4 Ms of age-friendly care and how to use them for the benefit of their residents:
- What Matters Most
- Medications
- Mentation
- Mobility
Attendees will learn how the 4 M framework aligns with existing nursing home processes and documentation procedures, including MDS and the consultant pharmacist role, as well as the pathway to gaining Age-Friendly Nursing Home recognition.
Learning Objectives
At the end of this session, attendees will be able to:
- Describe Age-Friendly Care and the 4 M’s with an Emphasis on Medications
- Identify the Domains of the Age-Friendly Hospital Measure
- Define the Path to Being Recognized as an Age-Friendly Facility
