Falls remain the leading cause of accident-related citations in long-term care and are the most common fact pattern associated with F689, Free of Accident Hazards, Supervision, and Devices. This program provides nursing home leaders with practical strategies to reduce fall risk, strengthen regulatory compliance, and build a defensible fall prevention program. Participants will review federal requirements, Delaware-specific regulatory expectations, post-fall investigation techniques, root cause analysis, individualized care planning, documentation, Quality Measures, and QAPI strategies. Attendees will leave with practical tools that can be immediately implemented to improve resident safety and survey readiness.
Learning Objectives
Upon completion of this program, participants will be able to:
- Explain the federal and Delaware regulatory requirements related to fall prevention and F689.
- Conduct a comprehensive post-fall investigation and identify contributing factors through root cause analysis.
- Develop individualized interventions and care plans based on identified fall risks.
- Describe documentation and MDS coding practices that support regulatory compliance and Quality Measure accuracy.
- Apply QAPI principles to identify trends and implement sustainable fall prevention strategies.

Mandy Smith is Vice President of Consulting Services at The Kemper Company, a long-term care consulting firm based in Ohio. She brings more than twenty-five years of experience in long-term care, including fifteen years as Regulatory Director at the Ohio Health Care Association, where she developed the CEAL and CEHCH certification programs. Her consulting work today spans regulatory compliance, survey readiness, clinical and operations support, QAPI, dementia care programming, and assisted living advocacy, with a clinical background as a licensed physical therapist assistant.
