Ombudsman Visits

Unannounced visits from the Ombudsman’s Office are happening in both SNF and ALF.
The ombudsman representatives are looking at/asking about the following areas:
- They are looking for grievance/concern forms
- Assignment sheets, and they are talking with the residence about their care delivery.
- New Ombudsman signs posted in multiple languages throughout the facility.
- Nursing schedules posted on the units.
- The facility has procedures for communicating with residents who don’t speak English or are visually impaired.
- The facility has regular resident council and residents are able to access the minutes.
- Menus are posted and residents have multiple menu options offered to them at each meal.
- Computers have screensavers so that private resident information is not out for everyone to see. Papers with resident HIPAA information are not left out for anyone to see.
- They ask if the facility has a family council.
- They ask who the grievance officer is and how the grievances are managed.
- They will interview residents to obtain feedback on activities & care delivery.
