Ombudsman Visits

Unannounced visits from the Ombudsman’s Office are happening in both SNF and ALF.

The ombudsman representatives are looking at/asking about the following areas:

  • They are looking for grievance/concern forms
  • Assignment sheets, and they are talking with the residence about their care delivery. 
  • New Ombudsman signs posted in multiple languages throughout the facility.
  • Nursing schedules posted on the units.
  • The facility has procedures for communicating with residents who don’t speak English or are visually impaired.
  • The facility has regular resident council and residents are able to access the minutes.
  • Menus are posted and residents have multiple menu options offered to them at each meal.
  • Computers have screensavers so that private resident information is not out for everyone to see. Papers with resident HIPAA information are not left out for anyone to see.
  • They ask if the facility has a family council.
  • They ask who the grievance officer is and how the grievances are managed.
  • They will interview residents to obtain feedback on activities & care delivery.
